Utilizing our Integrated Care Management (ICM)® model, we coordinate, implement, and monitor all of the services necessary to improve the quality of care and the quality of life of the people we serve.  Although there may be no cure for Alzheimer's disease, good planning and strong collaborative efforts by family members,  caregivers, medical professionals, social management teams, and community organizations, can ease the burdens on the patient and their family.
